We rank on six tests, each of which exists because a published figure failed it: captured at source, publishes a maintenance-dose price, one published structure, names its pharmacy, answered all four disclosure questions, and no withdrawn figure. Of thirty providers, one passes all six.
Why is this not ranked on price alone?
Price is one field, and it is the field most likely to be wrong. We have recorded a provider's reported price moving 66% in five days, another publishing six simultaneous structures selected by landing page, and a headline figure that turned out to belong to a different molecule entirely.
So the ranking runs on properties a reader can check without taking our word for anything.
How does every provider score on the six tests?
Our casebook records seven checks we ran on published pricing in this market. Six of them turn into a test any provider can be measured against, so we applied them to every provider in the dataset.

Which provider suits which priority?
- Maximum weight loss: tirzepatide. SURMOUNT-5 put it at 20.2% against semaglutide's 13.7% over 72 weeks.
- Cardiovascular risk in the picture: semaglutide. SELECT reported a 20% reduction in major adverse cardiovascular events; tirzepatide has no completed equivalent in that population.
- Lowest twelve-month cost: semaglutide, $1,740 against $2,232 on captured flat plans.
- You want an FDA-approved product: the oral Wegovy tablet at $149 or Foundayo at $149. Both have their own trial programmes. Nothing compounded does.
What can no ranking tell you?
Whether a specific vial contains what its label says. Whether sterility practice met USP <797> on the day your batch was made. Whether a prescriber genuinely read your intake. No desk review establishes those, and a site implying otherwise is overstating what it does.

No other provider's price has been captured by this publication. Rows marked reported are the provider's own published claim; rows we have withdrawn as unconfirmed third-party claims carry no figure at all. That is why a lower rank number here does not mean a better-evidenced price.
Ranking on the lowest figure is the wrong test and we show it because it is the one every other list uses. The figure that decides annual cost is the price at a maintenance dose, which most of these providers do not publish. Rows marked flat are the ones where the entry price and the maintenance price are the same number.
